COLA Living Pattern Survey (LPS) for the Uniformed Services 
U.S. Department of Defense 


Purpose of the Survey
This survey is designed to gather information on shopping habits of the typical military personnel at a duty station. Information collected in this survey will be used to research a variety of topics, including use of commissaries and exchanges in the computation of cost-of-living allowances.

Time to complete   
Please take approximately 15 minutes to complete this survey and answer all questions. 

Who should participate?
Service members (or spouses) assigned to a permanent duty station and have been residing at the duty station for at least two months.


Participation
Participation in this survey is voluntary. Failure to respond will not result in penalty to the respondent; however, maximum response is encouraged so that data will be representative of military shopping patterns in CONUS. Individual response will be treated as confidential. Only group statistics will be reported.
